Injection site reactions (redness, swelling, pain, itching) are the most common side effects, occurring in up to 8.5% of users.  Other frequent adverse effects include joint pain (arthralgia) at 13.3%, muscle pain, nausea, and peripheral edema (swelling of extremities). 

Serious side effects require immediate medical attention and include severe allergic reactions (rash, hives, swelling of the face/throat, difficulty breathing), hyperglycemia (high blood sugar), and carpal tunnel syndrome (numbness or tingling in hands).  Tesamorelin may also increase the risk of cancer and cause cardiovascular issues such as palpitations or fast heartbeat. 

Long-term or metabolic risks involve impaired insulin sensitivity and elevated IGF-1 levels, which can exacerbate conditions like diabetes.  Users may also experience mental health changes such as depression, insomnia, and headaches.  Discontinuation due to adverse events occurs in approximately 5% of patients, primarily due to musculoskeletal pain or injection-site irritation.

The standard FDA-approved dosage for tesamorelin is 2 mg once daily administered via subcutaneous injection for the reduction of excess abdominal fat in HIV-infected patients with lipodystrophy. 

In clinical studies and general medical practice, the dosage typically ranges from 1.4 mg to 2 mg daily. Key administration guidelines include:

• Frequency: Once daily, often taken before bedtime to align with natural growth hormone pulses. 

• Administration: Subcutaneous injection into the abdomen, rotating sites to avoid scar tissue or the navel. 

• Reconstitution: Egrifta vials must be reconstituted with the specific diluent provided (e.g., 0.5 mL for the 2 mg/vial formulation). 

• Off-Label/Peptide Community Protocols: Some users and clinics employ lower doses (e.g., 1 mg to 1.5 mg) or cycling schedules (e.g., 5 days on, 2 days off), though these are not FDA-approved indications. 

Tesamorelin is not indicated for general weight loss management in the absence of HIV-associated lipodystrophy. Always consult a licensed physician for individualized dosing and medical advice.
